New/Change of Direct Debit Instruction Notification Form

Direct Debit Authorisation

Notify an addition or change to a direct debit authorisation for your MPF scheme or a particular pay centre, and authorise deduction of MPF contributions from your bank account.

 

Paying your contributions through direct debit saves you time in writing and checking the payee and total amount for each payment.

 

Please note that about 2 weeks' processing time is required to set up a direct debit authorisation for a bank account with HSBC or Hang Seng Bank, or about a month for other bank accounts.

Change of Investment Instruction Form

Change your investment instruction of your existing investments and/or future contributions and/or accrued benefits transferred from another Registered Scheme. This form is applicable if you want to:

  • Change the investment allocation of your existing investments, future contributions and accrued benefits transferred from another registered scheme. Appropriate units in your account will be redeemed (if applicable) in order to purchase other funds to meet your new investment allocation percentage; or
  • Change the investment allocation of your future contributions and accrued benefits transferred from another registered scheme only. The investment allocation of your existing investments will remain unchanged; or
  • Change the investment allocation of your existing investments only. The investment allocation of your future contributions and accrued benefits transferred from another registered scheme will remain unchanged; or
  • Exit the Default Investment Strategy (DIS). All of your existing investments, future contributions and accrued benefits transferred from another Registered Scheme will invest per the investment allocation immediately before you exit the DIS. To avoid doubt, no subsequent rebalancing or de-risking of the investment allocation will be done with regard to existing investments, future contributions and accrued benefits transferred from another registered scheme.

Duplicate Remittance Statement Requisition Form

The handling charge is HKD50 per statement for a minimum of HKD200 per request.

Account Statement Requisition Form

1.  (Applicable to employer or member) Request a copy of any previous employer or member statement for up to 7 years prior.
The handling charge is HKD50 per scheme financial year for each copy.
2.  (Applicable to employer) Request a Member Contribution History Report.
3.  (Applicable to employer) Request for provision of information of employees’ accrued benefits for the purpose of processing the refund or calculation of long service payment/severance payment paid or payable to the related employee for the purpose of Section 12A of Mandatory Provident Fund Schemes Ordinance.
